A fissure at 3 oclock is a bit unusual but at least a colorectal surgeon should evaluate. Narrow stools with a fissure suspicious for spasm in the anal sphincter and this could lead to infrequent and difficult stools. You can gall but cutting back on caffeine and alcohol increasing your intake of other fluids. If a surgeon suggest a proctoscopy or sigmoidoscopy &/or sphincterotmy, get it done.
