You can take an OTC NSAID such as ibuprofen or naproxen OTC during the flare up but you also need to take allopurinol for avoiding recurrence. Allopurinol is cheap with cash pay but it is not OTC so you should go a clinic for indigents for the care. There is no effective supplement OTC to replace uric acid lowering medication such as allopurinol (cheap) and febuxostat (expensive).
