Small amount of fluid in the Douglas cul de sac (the rectouterine pouch) is not a rare finding when patient has pelvic ultrasound and this finding is usually benign. By radiopaedia.org: "a small amount of homogeneous and low-attenuated fluid in the rectouterine pouch is normally seen in child-bearing age women". Take care.

The findings of small amount of fluid in the Douglas pouch are not explaining or are not diagnostic for your abdominal pain. Work up should continue in trying to explain what might be causing your symptoms, best coordinated by current doc.
