I have varicocele and i was at urologist 5 months ago and i have appointment next month. everything was fine on first visit but now i think my testicle has shrinked. is it too long to wait for next month? can atrophy cause problems before next visit?

Varicocele alone will not shrink your testicle in such a short period and waiting one month will not change things. Your assessment is probably in error. In the meantime you can see your family doc to check you
