Hospitals and inpatient reviewers justify length of stay based on the codes that come up from chart reviews. Reviewers look at charts daily in many places. NO CODE/NO COVERAGE. Unfortunately the healthcare system is evolving into a mess where the bean counters get more attention than a patients needs or a doctors clinical opinion.

Not decisions, but suggestions that can significantly influence billing, a model that the majority of inpatient facilities have embraced in order to stay afloat financially. Physicians still decide if suggestions appropriate or not, unfortunately most of the time approving them due to the intrinsic conflict of interests.
