It means the urine dipstick test detects heavy (3+) occult blood. However, please note that dipstick test uses reagent to detect hemoglobin ( a chemical test) so positive occult blood is not always equal to the presence of red blood cells in the urine. Microscopic exam needs to be done to confirm the presence of red blood cells, i.e. true hematuria. More than 3 red blood cells/hpf is abnormal.
