Why would a dentist choose not to replace a temporary crown that has a small hole on it?

Temporary restorations are just that... temporary. A small defect on the temp will have no bearing on the final restoration nor will it cause damage to your tooth in she short time that it is in your mouth.

It's a "temporary" crown, designed for only short term use. Have the permanent crown placed ASAP and you should be fine.
