I wonder what you mean by "notice". For the sx, chronic cough, low grade fever in the evening/night, poor appetite and weight loss. Physical exam is less like useful unless your doctor are highly experienced with TB. A recently converted PPD (tuberculin) test & positive interferon-gamma test + chest Rx studies are reliable for detecting the lesions and finally sputum work-up for confirmation.

History of exposure, symptoms including pulmonary, such as cough and systemic, fever and/or weight loss, for example, a physical exam and testing are usually part of the initial diagnostic process. There are tb forms other than pulmonary that are in general harder to diagnose and affecting other systems, renal or gastrointestinal, for example.
