A wide QRS indicates a delay in conduction from the normal pacemaker in the atrium ( upper chamber) down through the heart's electrical system which causes delay in the contraction of the heart's ventricles( lower chambers). There are numerous reasons for a wide QRS and they are all abnormalities like arrhythmias- SVT, WPW, medications, electrolyte abnormalities like hyperkalemia, acute MI. These are only a few.
