At "45" , fertility is reduced considerably due to age of eggs, and even more unlikely given a "blocked tube" and "endometriosis". Suggest a consult with a reproductive endocrinologist, who may recommend egg retrieval and intracytoplasmic sperm insertion (ICSI) for fertiliztion, with embryo transfer, and/or ovulation induction.

It is arguable if laparoscopic surgery would help you conceive even if you were 20 years younger, and any marginal benefit could easily be negated by decreased fertility due to any delays at your age. In vitro procedures are by far your best option, but if that is not something you are able or willing to pursue you may discuss laparoscopy further with your doctor or via a second opinion consult.
