My cardiologist says that 75mg irbesartan is equal to 40mg valsartan... doesn't make sense, i want to try irbesartan but don't want to experience hypotension on first dose. bp on 40mg valsartan around 115-120/78?

I think you asked this question before. Lowest dose of Irbesartan is 75 mg (75, 150, 300 mg) and lowest dose of Valsartan is 40 mg (40, 80, 160, 320 mg) so those noted doses are comparable and you should not pay attention to the total mg in each tablet because the formula is different. If you have any doubt, you can try 1/2 of irbesartan for a couple weeks (2 weeks at least) then check BP.
