You don't say you're sad, overwhelmed, or give any reason for committing suicide, and you say self-harm will also satisfy these urges. Try to be objective and ask yourself if you deserve this self-punishment. If you truly want to end your life, don't kill yourself; start a new life. Get on a plane or boat to a very different place and start over. I'm serious, but only if death is the alternative.

If you are not confident with your therapist please see a Clinical Psychologist with expertise in multiple types of psychotherapy to diagnose these suicidal thoughts and formulate a treatment plan which may include medication prescribed by a Psychiatrist. Also, please have a complete physical exam to determine if some of your symptoms are due to a physical problem.
