Ct urogram showed "mild bilateral perinephric fat stranding, no suspicious solid renal mass, intrarenal calculi, caliectasis or hydronephrosis bilaterally. urologist recommends cystoscopy. is this a little too invasive at this moment ?. thank you?

No, it is absolutely indicated because patient has obstructive uropathy and/or nephropathy. I strongly suspect kidney function is not normal or it will significantly deteriorate soon if obstruction is not relieved. Cystoscopy needs to be done ASAP.

Bilateral fat stranding as an incidental finding in an asymptomatic pt needs no treatment. Stranding takes place in some people with acute kidney infection (acute pyelonephritis) which is symptomatic. Cystoscopy means looking inside the bladder and outer urinary passageway and is not related to this finding in any way. If still in doubt get a second opinion. Youll be OK
