Appears that you possess cervical spondylosis with possible radicular nerve compression at C6-7. The twitches in triceps are most likely fasciculations. Surgery is best done if progressive weakness, atrophy, unremitting pain, urinary issues, spinal cord compression. Otherwise, physical therapy, with motorized cervical traction may help substantially. An epidural steroid may help temporarily.
