If the liver is enlarged, the ultrasound report should indicate it, especially you had US in the past before the cholecystectomy. A slightly high ALT is not much concern but it should be followed up for making sure not getting worse or back to normal with. Liver hemangioma is not uncommon, less likely causing the pain, especially if it was present with previous ultrasound.
