The common heart issues for a premature at that level are easily detected/monitored and fixed by mid childhood if they don't resolve naturally. The more worrisome ones effect the brain and motor development. (Cerebral palsy). I can't see any kid making it to adulthood that has a neonatal problem that would explain such a vague symptom.
