My sister is in an indiana hospital for icd surgery but is not a candidate due to not being clean of drug use for 6 months. is this a state or federal law or just hospital policy? where can she possibly get the surgery? her heart is working @ 11%?

Due to active drug use, your sister could fall in this category of contraindication for ICD: ICD therapy is not recommended in NYHA Class IV with drug-refractory congestive heart failure who are not candidates for cardiac transplantation or CRT-D. I do not think there is such law, only guidelines for indications and contraindications.
