A single dose of 2 grams (500 mg x 4) has a high frequency of abdominal discomfort, nausea, vomiting, and diarrhea in the next few hours after taking it. Aside from these symptoms, it is entirely harmless.

2g Azithromycin or 4 x 500 mg pills is the recommended dose in specific conditions, that is typically used to increase compliance in an one time dose. Side effects remain possible, not commonly seen otherwise.
