In that region there is muscle, spleen,colon,stomach& pancreas. Gas can accumulate there in the colon. Gastritis,( inflammation of stomach lining) can be a source of tenderness. After an exam by your MD you can be advised as to possible causes of tenderness & whether any testing or referral to a gastroenterologist would be advisable
