Suggest it is time to have a second opinion from a neurologist, as you need a specific diagnosis, and appropriate therapy for the condition. Tension cephalgia, primary cervical issues with secondary headaches, thoracic outlet syndrome, and greater and/or lesser occipital neuralgia, are some of the possibilities.
