You say nothing about follow-up care post-hospitalization. You are overdue for a return visit to your GP for appropriate care.

Time to see your doc for a thorough exam and bloodwork. Your symptoms are suggesting early congestive heart failure, among many others with uncontrolled diabetes at baseline. The sooner you seek medical attention, the better as it may prevent a possible ER visit.
