Specialists: You don't mention your age, but if you're on the young side, your syndrome appears to have features of a vasculitis in view of the TIA history, lung symptoms and ANA titer. For this reason I recommend having a rheumatologist involved in your care. I wonder if a biopsy of your lymph nodes has been done to check for systemic vasculitis or whether the nodes only show nonspecific reactive changes.
