6 year old gets same hickey looking bruise on his upper arm every few months. once it switched to left arm but other times is on right. it fades in a couple days but he can never recall doing anything to bring it on? should i be concerned?

I would suspect someone is abusing him. Or he falls a lot or gets into fights. Check all these possibilities. Please.
