For primary FSGS (confirmed by biopsy), we treat with immunosuppressive medications which include steroids and calcineurin inhibitors (cyclosporine or tacrolimus) if patient has nephrotic syndrome. If your proteinuria is low (urine protein/creatinine is less than 0.5) I will continue only Telmisartan and try to maintain systolic pressure less than 130. Are you sure you have had primary FSGS?
