What is the first approach for primary fsgs my kidney gfr is between 36 to 42 i'm 78 years and protein urine is between upper normal between 28 to 31 mg / dl and no other remarkable tests regarding kidney function i'm taking telmisartan for ess. ht?

For primary FSGS (confirmed by biopsy), we treat with immunosuppressive medications which include steroids and calcineurin inhibitors (cyclosporine or tacrolimus) if patient has nephrotic syndrome. If your proteinuria is low (urine protein/creatinine is less than 0.5) I will continue only Telmisartan and try to maintain systolic pressure less than 130. Are you sure you have had primary FSGS?
