No. Empty sella is not that rare and and it can be associated with increased intracranial pressure. Herniation of 3rd ventricle into empty sella is much more rare. It almost always may occur with secondary enlarged sella and inflammation. Empty sella may not need treatment or at most medical treatment, while herniation of third ventricle usually with optic chiasm herniation need neurosurgery.
