So doctors know immediately when a new fda approved drug/vaccine is available for whatever disease? or do they go to conferences,word by mouth from other physicians etc, or know before it is even approved and available to then population?

Doctors know ahead of FDA approval if they are experts in the field of the drug's use and they are aware of the current research on the drug or therapy involved. Otherwise, they read about the new drug or therapy FDA approvals, or perhaps hear about them at meetings.
