If you have a history of low back pain, and you "barely walk now", this really needs to be handled ASAP. Myofascial Pain is a rather unlikely answer, and lumbar stenosis seems possible. Updated MRI can illuminate, physical therapy will help, and a physiatrist would be a good choice.
