While our hope is to maintain seizure freedom on one AED, at the right dose, success is limited to roughly 50%. It’s not uncommon to have been seizure free for several months on an antiepileptic drug (AED) before having breakthrough seizure. Your body’s Lamotrigine metabolism may be changing or one of several other things could be happening as well. I advise seeing your Neurologist for discussion.

Key question to ask is whether Lamictal level is within an ongoing therapeutic range, and this could be easily adjusted. That is the most likely answer to "breakthrough", assuming no supervening disease co-morbidity. If appropriate serum level, then a second AED should be added.
