Do you know why some people have ems perform cpr on them for 20 minutes or longer after sca, and they have very little to no brain damage and recover. others have ems perform cpr on them for around 8 to 10 minutes and have severe brain damge?

Each case is different than the next. There is no single answer. But the main difference likely is the duration of the event before CPR was started. And CPR is just more effective in some people than others. For example, getting satisfactory circulation with chest compressions is a lot more challenging in obese than slim persons.

Duration of the CPR cannot be used prognostically, as there is biological variation and different presence of co-morbidities and medications. Pre-existing subclinical deterioration will also play a substantial role.
