Atherosclerosis.: Most likely reason for these changes is hardening of blood vessels in the brain resulting in decreased blood supply to the brain tissues. Such changes are seen quite often in older people and may not be causing any symptoms. However they may be seen with vascular dementia and stroke. They are irreversible.

"White matter": I agree with dr. Kumar. "white matter" refers to the nerve fibers that facilitate communication between nerve cells -- along with myelin, a fatty sheath that covers the them. White matter is below the surface layer of the brain, and can be followed all the way down the spinal cord. Strokes can injure these areas; some people have cognitive changes as well as physical symptoms afterwards.
