This question isn't for me but what does it mean for someone with copd 4th stage , cad, respiratory failure , osteoporosis to be coughing up blood ?

Get attention, perhaps a visit to emergency dept. Coughing up blood may well be a sign of impending deterioration, especially in complex medical status described. This indeed is a potential sign of decompensation, and must be heeded. Good luck.
