I had intra-articular fracture of both heels over a year ago and now i have arthritis in both ankles. pain is agonizingly high and my question is, will the pain progress and get worse with time or is it going to get somewhat better?

Physical therapy....: This is rather difficult to predict, especially given your young age. But, you can go to physical therapy and learn different exercises to do that will hopefully prevent further progression of the arthritis over time. The physical therapists can also see if any type of splinting , or other modality, would be helpful to decrease your current pain. Good luck to you!

Progressive.: Fortunately you are young, but you have been diagnosed with arthritis. I would follow your PCP advise and avoid excess strain. A topical cream may help., such as voltarin.

Post injury pain: My policy in such cases of post traumatic & postoperative chronic and severe ankle problems is to refer them to specialists in this type of problem at the orthopedic department at a university hospital near you. The longer you go on with these symptoms the less corrective measures will be successful. Hopefully you'll be spared many years of chronic pain & disability with proper treatment.
