Only saw base: I'm sure the report said the "bases are clear" , meaning just the very bottoms of the lungs that can be seen incidentally on an abdominal CT. You would not know anything about the remainder of your lungs, so if you're concerned about lung cancer, you would indeed need to have a chest CT to look at all lung fields, not just the bottoms.

CT scan: CT abdomen sometimes includes the lung bases but it is not dedicated to that. If need to get lungs evaluated then need a dedicated lung CT or chest xray based on what you are looking for.
