May have a capsule: If there is a change in the implant, it is very possible that you have a capsular contracture, which is thickening of the scar tissue around the implant. This can cause distortion of the implant and associated pain. You should see your plastic surgeon for an examination.

May need new implant: It is possible that the implant has ruptured and or you have capsule contracture (or scar tissue around the implant). This is a very common scenario with implants. You should have your surgeon evaluate you. You may need an imaging study such as an MRI or ultrasound. You may need the implant replaced.

Breast cancer : Breast reconstruction performed with the use of tissue expanders followed by expansion and later exchange to permanent implants is still a complicated procedure. Even if everything goes smoothly there is scar formation around the implant that over time can become so firm that it is painful or that can result in a change of the appearance of the implant. A diep flap may solve this problem.

Impossible to know..: Please see your plastic surgeon or go to another that you trust or is recommended. Without seeing you, it's impossible to know but i would recommend sooner rather than later.

Needs evaluation: Without at least a photo, it's hard to give you a specific answer. It might be a deflation of your implant. It could be a capsule has formed. See your reconstructive surgeon for an evaluation.
