For most people: There is not much difference between saline and silicone implants. All implants are wrinkled, but saline seem more wrinkly because they are filled with salt water. Silicone implants are filled with a gel that is thicker, so the wrinkling is not as apparent. This is only an issue in very thin patients, where the wrinkling may be noticeable (but usually not in clothes or a swimsuit.).

Both: Effective. Surgery about the same depending on size chosen. Talk with surgeon about risks, benefits, and options. Early silicon had some leakage problems, but rare now.

Both/ neither: While both are "effective" at making your breasts larger, silicone implants are far superior to saline in a number of ways. The pain involved is mostly related to whether the implants are placed in front of the muscle or behind the muscle. If using silicone, because of the natural feel and lack of ripples, it is possible to place the implant in from of the muscle, so less painful!
