Discomfort: Introducing a speculum to a tight vagina can be very uncomfortable and inflamation of tissues could cause some mild symtpoms afterwards.

Try anesthetic gel: Pelvic exams should no longer cause "extreme pain".(period). If there is a clear necessity or a strong clinical indication for your "pelvic exam with a speculum", ask a skilled and caring doctor/clinician to please use a local anesthetic (like Lidocaine cream or gel) on your vulva and inside your vagina several minutes before attempting to insert the "smallest speculum".
