Doctors say iugr fetus are small for their age in utero, does this mean that the fetus is just a small version of healthy fetus same age? or can this mean limbs etc. arent seen yet on ultrasound and the fetus is delayed by anatomy wise too

Small means small: Essentially all fetuses have all external parts formed by 12 wks, The Small for dates label is applied to those who are not putting on weight/size as expected for the stage of pregnancy. There are various potential causes from a poor placental attachment, maternal high blood pressure, metabolic issues,infection & many others.
