See dermatologist: It's not uncommon for a 4 y.o to scratch one particular spot. Could be just nervous habit called lsc or might be seb dermatitis or focus of eczema. would see dermatologist for accurate dx and poss tx. a steroid cream or lotion might be very helpful for the itching.
