I have a scab on my head, it's been there for years, doesn't bleed and occasionally itches what might it be?

Irritated lesion: It can be many things. Do you pick at it? It could be an inflammed mole, wart, folliculitis (infected hair follicle) to name a few. Has it grown? If you are not picking at it and it does not heal, have your doctor check it.

It may not be a scab: It sounds like you have an area on your head that may feel scaly or rough. This could be from a skin condition known as seborrhea or dandruff, which can cause dry patches and flaking. Or it could be some kind of mole. Or some other skin condition. Its hard to know without seeing it first. Its a good idea to have it looked at by a doctor. Best of luck
