i have a scab on my head, it's been there for years, doesn't bleed and occasionally itches what might it be?

Irritated lesion: It can be many things. Do you pick at it? It could be an inflammed mole, wart, folliculitis (infected hair follicle) to name a few. Has it grown? If you are not picking at it and it does not heal, have your doctor check it.
