What can cause a shocking pain in right testicle in 12 year old boy? no swelling.

If severe go to ER: Severe pain can be associated with torsion of the testicle..that is an emergency. Other causes can include inflammation, trauma, epidydimitis, orchitis etc. So my advice is if severe please have him seen and consider Ultrasound with doppler studies for blood flow.
