Confusing question: I don't recall ever having a 3 yo with testicular swelling and if so would do an ultrasound and likely send him to a pediatric urologist.If you have notices the scrotal sac become fuller, that is much more common and could represent water or a hernia. Either way, a visit to the doc is reasonable.
