The transition age: Hospitals do not generally define the age of transition the doc who treats you does. Many young adults with chronic conditions or close relationships with their doc/treatment team will remain into their early twenties. This includes their treatments if needed at a children's hospital. Those in normal health or changing docs transition to adult specialists by their late teens. Discuss with your doc
