Anemia: I would be very concerned with her iron deficiency and fatigue. I would be concerned about colon cancer. I would recommend evaluation by a physician. She should have a complete exam, blood tests and I would consider colonoscopy and possible endoscopy.

Blood loss: If she is post-menopausal, it is imperative to ascertain if she is bleeding in her intestine and a colosccopy is warranted at her age. She should take oral iron and multivitamin supplements to correct her iron deficiency if it is a hang over from her days of excessive blood loss from menstruation, or if she has on-going excessive bleeding during periods.
