Hard to tell...: If there is a tonsil infection, and it is a bacterial infection like strep throat, one will have started antibiotics. If pain is worse, such as having more pain, difficulty breathing, difficulty swallowing, or too much cough, then it is hard to tell what is going on without another exam. That means seeing the doctor again. Bad things like a big tonsillar abscess or throat abscess can happen.
