Colitis: Need more info but this could be colitis (inflammation of the large bowel) - many causes eg inflammatory bowel disease like Chrons or Ulcerative Colitis (more likely if you have mucous as well), infections eg campylobacter/salmonella. Diverticulitis is possible. Would be an unusual presentation for appendicitis. Best to see a Dr for detailed history + examination +/- blood tests.

Blood in stool : Blood in stool with abdominal and back pain can be due to various reasons like anal fissure , haemorrhoids, inflammation ( inflammatory bowel disease , colitis ) etc . Need more history , please consult your doctor.
