Swimmers ear Rx: Azithromycin (Zpack) is very long acting; it is in the system for 10 days after the last dose. Therefore, failing to improve or getting worse probably means a different antibiotic is needed or that no antibiotic will work (e.g. virus, allergy). Swimmers ear usually is a bacterial infection, but the most common causes don't respond to azithro. Discuss with the doctor treating you.
