Hyperthyroid: Your FT4 is high, TSH low, positive antibodies, all consistent with autoimmune thyroid disease, such as Graves disease, or possibly Hashitoxicosis. Common symptoms are tremor, palpitations, anxiety, feeling hot, and others. I suspect your whole thyroid is enlarged, although the R lobe may be bigger. You need to see an Endocrinologist. Medications (e.g. methimazole) now. Maybe ablation later.

Solitary: Hyper functioning thyroid nodules are considered benign. The use of methimazole or treatment with Radio iodine treatment will be the likely options. If treatment with either of the above surgery may be an option
