ER: I would take your boyfriend to the ER. All of the above infectious diseases are treated with Doxycycline, however, with his neurological sypmtoms, a stroke needs to be ruled out with CT scan and a MRI with Magnetic Resonance Angiogram and EEG in addition to expert consultation with a Neurologist, full history and physical exam are indicated.

Tick bite: Neurological sx's from Lyme Dz can start within days of infection. Was there a delay in treatment? Was he tested for Anaplamosis? Fortunately, Lyme, RMSF & Anaplasmosis can all be treated with Doxycline. If sx's are not abating would recommend that he be followed by someone who is experienced with Lyme Dz treatments and complications (such as infectious Disease specialist)
