See Ortho: Studies show that the bony protuberance on the outside of your ankle is called the lateral malleolus and it is part of your fibula, dont worry, its supposed to be there, you can google ankle anatomy to confirm this. If you are having achilles pain it is most likely tendonitis, less likely however possible is the need for an exam to rule out enthesopathy associated with ankylosing spondylitis.

Ankle issues.: When your ankle hurts to point, it usually means something is amiss. Achilles tendon pain could possibly mean that it is involved also. Bones sticking out with pain are good indicators that you need to be examined by a specialist to see what's what.
