Tailors bunion: bone spur or tailors bunion deformity. The shoe rubs on the skin and the skin rubs on the spur causing the pain. As we age the bone or knuckle gets bigger. Wider shoes can help or surgery to remove the spur can help. See your local podiatrist

Somethings got to go: I'm not sure just what you mean when you say "something" is sticking out. Is it bone or possibly a foreign body. A foreign body would have to be removed to give you relief. The bone might be treated conservatively. it really depends on the actual findings on the x-ray.
